ON-SITE

On-site registration will open at 1:00 PM on Tuesday, March 26, 2011. A JPY5,000 surcharge will be added to your registration for on-site processing fees. Check here for the on-site registration hours and information.

For Japanese attendees: Fellows, Residents and Students need to submit an identification form. Click on the button below to download the form.

For American attendees: Fellows, Residents and Students need to provide a copy of ID to the Secretariat by E-mail (5jassh@congre.co.jp) or
FAX (+81-3-5216-5552) by Feb. 28, 2011.

** Children ages 15 and under may attend without paying registration fees.

• The registration fee includes admittance to scientific sessions, symposia, key-note lectures, poster exhibits, commercial exhibits, opening ceremony, welcome reception, closing ceremony, continental breakfasts from Sunday through Tuesday and a copy of the abstract book.
• The registration fee for accompanying person includes admittance to the opening ceremony, welcome reception, closing ceremony and continental breakfasts from Sunday through Tuesday.
• Additional fees apply for the social events such as Garden party on Mar. 28 and the sports activities.

* Accompanying persons who are also healthcare professionals (doctor, nurse, hand therapist) must pay the appropriate registration fee in the appropriate professional category if they want to receive continuing education credits.

We are planning to organize some sports events and product demonstration workshops. If you are interested to participate in these programs, please let us know in the registration form so that we can send you further information when everything is determined.

Cancellation must be made by informing the Secretariat by e-mail. The amount of refund will depends on the date of cancellation, as follows;

Confirmations will be e-mailed to you when your registration is processed. If you do not receive your confirmation, please email 5jassh@congre.co.jp to make sure it was received.

Badges, tickets and the abstract books will be available for pickup at the Registration Desk. These items will not be mailed to you. Refer to the below for the registration open hours and location.

NO CME WILL BE AWARDED TO INDIVIDUALS WHO HAVE NOT PICKED UP THEIR PACKETS
