All participants are required to register by completing the Registration Form. Receipt of the Registration Form will be acknowledged by e-mail. Upon receipt of the Registration Form and payment of the registration fee, a receipt and confirmation will be sent to you. For advance registration, the form and payment must be received by the secretariat by March 10, 2008.
*Residents/Fellows must submit an official university certificate or letter, in conjunction with their registration, which confirms their status. Documentation must be faxed to the Secretariat at: +81-6-6229-2556. Documentation must come from a supervisor, or department head.

All cancellations made in writing and received by the secretariat by April 30, 2008 will be eligible for a full refund, less a 5,000JPY administration fee. Cancellations received on or after May 1, 2008 will be 100% NON REFUNDABLE.
