Method of Payment: |

Payment must be made on-line by credit card when you register. Please give the name of the cardholder, card number and date of expiry. The following credit cards are accepted: Diners Club, VISA, MasterCard, American Express and JCB. |

Receipt & Registration Confirmation |
|
After payment of the registration fee, a “Registration Confirmation with Receipt” e-mail will automatically be sent to your registered e-mail address. Please print out this confirmation e-mail and present it at the registration desk on-site upon your arrival at the Congress venue. |

The transaction will appear on your credit card statement as "ISHAM2009." |

Cancellation and Refund Policy |
|
The secretariat office must be notified in writing of all cancellations.
In all cases, an administrative charge of 5,000 yen applies and will be deducted from the refundable amount. Refunds will be made to the same credit card used for on-line registration fee payment. Refunds will be processed after the congress as follows: |

|
In additional to a valid passport, some foreign nationals are required to obtain a Visa to enter Japan issued by the Japanese Embassy or Consulate in your country. If you need a visa, please check the “Required” box in the Visa section of the online registration form. Please also select your “Nationality” from the drop-down menu. The Secretariat will assist you with preparation of documents to submit with your application to the Embassy in your country. Visa documentation can only be provided for pre-paid Registration. |
