[Important Points When Leaving Your Child]

• Please bring your ID (health insurance card, driver’s license, passport, etc.)
• If the condition of your child’s health differs from usual, please notify us.
• We cannot accept your child under the following conditions:
  • If your child has a high fever or is otherwise in poor physical health on the scheduled day.
  • If PH determines that your child cannot be accepted for care.
  • If your child has, or may have, an infectious disease or other illness.

[Returning the Child]

• The same parent or guardian who dropped off your child should collect them. Be sure to present the Confirmation Form. We will then return the child and his/her belongings.

If a different person will be collecting your child, please notify the staff at the time of dropping off your child. The person collecting your child must also present ID, and should be indicated at the time of the application.

If we have not been informed of any change in advance, or in the event that the Confirmation Form has been lost, we will not allow a different person to collect your child, until we can confirm both the change of the collecting individual and their identity.

If the child is not collected within 30 minutes of the indicated collection time, PH may contact the Management Secretariat for JCS2019.

[Meals]

We will not make any food available to your child, save for what you provide, due to dietary considerations, as well as the possibility of an allergic reaction. In the event that what you provide causes food poisoning, adverse physical reaction, or similar, please note that the Japanese Circulation Society meeting and its preparation room can accept no responsibility.

[Emergency]

We will contact you at your emergency contact number in the event that your child has a fever or suffers an injury. Should immediate treatment be required, we will take your child to the hospital.
